PHP/MYSQL Web survey site

Specifications

Users should be able to log in, and access surveys only available to them. Results should then be stored in the database. An admin page should also be setup to add/delete surveys, add/delete users, view results, send results to relevent people.

Results should be displayed via the web or output to an excel file.

Deliverables

Seller agrees to deliver the following:

• Programs, documents, and/or web sites as specified by the buyer in the Specifications & Requirements section and in any attached files, along with source code for all work created.
• Fully tested deliverables. If the project is a web site or other hosted product, Seller will install deliverable into Buyer's environment and ensure that it runs at the time of delivery.
• Deliverables are "work for hire" and are the property of the Buyer.
• The Seller may not incorporate any open source, GPL, or other third party products into the deliverable without notifying and obtaining Buyer's agreement first.
